HOW TO PREPARE BEFORE YOUR VISIT

SCHOOL/HEALTH FORMS

Forms can be completed if the LAST WELL CHILD VISIT is within 1 year. Please allow up to 2 weeks for completion. Please indicate if needed sooner and we will do our best to accommodate you. Please put CHILD’S NAME & DATE OF BIRTH on all forms and be prepared to sign other forms for us to have the ability to send information (fax). When applicable, provide a self-addressed stamped envelope and complete PARENT SECTION first, otherwise completed forms will be sent to the Patient Portal or available for pick-up once completed. 



We do not do testing for COVID or FLU at our office; so we may ask parents to perform home tests, based on the child's symptoms & vaccine status, before performing an exam at the office.

Please read the attached Erie County measles health advisory.

  • Of note, CDC and NYSDOH recommend that all U.S. residents older than age 6 months without evidence of immunity who are planning to travel internationally and have no MMR vaccine contraindications should receive an MMR vaccine prior to departure.
  • For children 6 through 11 months of age, this dose is not considered valid, and 2 valid doses on or after the first birthday are required.
  • With spring/summer break approaching, families may inquire about a “travel” MMR vaccine dose for children 6 through 11 months of age who plan to travel domestically.
  • Given the level of measles transmissibility, a growing measles outbreak and MMR vaccine safety and effectiveness, although not within CDC and NYSDOH guidelines, healthcare providers could consider administering a “travel” dose prior to domestic travel for 6 thru 11 month olds who have no MMR vaccine contraindications after reviewing with parents risks and benefits and ensuring that parents understand that 2 additional MMR vaccine doses on or after the first birthday are required.
  • Although not consistent with CDC and NYSDOH guidelines, given current measles epidemiology, benefits of this practice outweigh risks.


NYS IMMUNIZATION REQUIREMENTS

Automated reminder texts/calls are still going out, please respond appropriately to avoid another call to you, from our office, to confirm the appointment. Appointments that are “missed” or are cancelled with less than 12 hr notice, will result in a $35 fee.  Confirmation calls/text are courtesy - please document your appointment.  Appointments are also listed on the Patient Portal. 


A Confidential questionnaire completed by the teen/adolescent is private between the patient and the provider. This is a measure of the "Best Practice" for Pediatric Practices.  This occasionally results in a "patient balance due" from your insurance carrier.


“We are ACTIVELY open” for all visits, but please try to come with minimal visitors/accompanying people.



Whenever possible, we recommend that you bring only the child(ren) who are scheduled to be seen in the office.



We have an excellent track record for prevention of disease spread due to separate entrances, waiting rooms, and exam rooms for our Healthy vs. Sick Patients.

Telemed Calls – Sick triage nurses and Providers may occasionally practice “phone medicine”, when portal pictures can be effectively sent. Please see the Patient Portal Info tab if there are portal questions.
